Job Description
Title: Physical Therapist

Schedule: Days, 8-hour shifts

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: Not specified

Requirements:
- Oklahoma State License (Required)
- 1 year of Physical Therapy experience (Required)
- LTC or Rehab experience (Required)
- New Grads OK with Level II experience

Roles & Responsibilities:
- Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care, developing an appropriate plan of care.
- Utilize standardized assessments and evidence-based practice to support clinical interventions.
- Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, and wound care.
- Assess patient progress, adjust treatments, and document all required information in the electronic system.
- Provide consultation and counseling to patients, families, and other service providers regarding physical disorders.
- Communicate effectively with supervisors and healthcare team members about patient progress, barriers, and treatment plans.
- Participate in meetings related to patient care and required trainings.
- Supervise and support physical therapist assistants and students as needed.

Compliance / Notes:
- Adherence to APTA and state practice acts, code of conduct, and ethics during professional practice as a Physical Therapist.
