Title: Physical Therapist

Schedule: Shift: 5x8 Days

Assignment Details: Guaranteed Hours: Not specified, Contract Weeks: 13

Requirements:
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the state of practice
- Experience in skilled nursing facility or geriatric rehabilitation preferred

Roles & Responsibilities:
- Conduct initial and ongoing assessments of residents' mobility, balance, strength, gait, and functional limitations
- Develop and implement person-centered treatment plans to enhance physical function and prevent disability
- Provide direct therapy interventions, including therapeutic exercises, gait training, and balance retraining
- Monitor residents' progress and modify care plans as necessary
- Educate residents, families, and caregivers on home exercise programs and safety strategies
- Supervise Physical Therapist Assistants and therapy aides as required by licensure
