A clinical infusion pharmacist is essential in managing complex intravenous (IV) and subcutaneous therapies in a home care setting. This role involves preparing sterile compounded medications, adjusting dosing based on clinical data, and providing thorough patient education. The pharmacist acts as a key member of a multidisciplinary healthcare team, ensuring the highest standards of therapy management and patient safety.

Location: Glendale, AZ
Contract Duration: 8-12 weeks
Schedule:

• Monday through Friday, 8:30 a.m. to 5:00 p.m.
• May require participation in on-call rotation

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in a sterile environment adhering to state and federal guidelines.
• Monitor lab values, calculate pharmacokinetic dosing (e.g., vancomycin), and manage TPN regimens to tailor patient treatment plans.
• Provide detailed education to patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance.
• Collaborate with physicians, home health nurses, and internal teams to optimize therapy, reduce hospital readmissions, and support seamless patient care.
• Ensure compliance with regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Maintain accurate inventory control of medications, including temperature-sensitive products, and oversee safe packing and shipment to patients' homes.

Desired Qualifications:

• 1-3 years of experience in infusion specialty pharmacy or hospital pharmacy IV room operations.
• Familiarity with IVIg therapies is advantageous.
• Understanding of specialty therapy payer requirements, reimbursement processes, and contractual obligations.
• Experience with CareTend pharmacy system preferred.
• Current and active Arizona pharmacist license in good standing.
