Position Overview

We are seeking a skilled Social Worker with expertise in both social work and discharge planning within an acute care environment. The ideal candidate will have a minimum of two years of acute medical social work experience and will play a critical role in providing effective care coordination and support for patients and their families.

Job Details

• Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)
• Start Date: ASAP
• Weekend Requirements: Typically one full weekend every four weeks, subject to departmental needs
• Patient Ratio: 1 social worker to 25 patients
• Patient Load: Up to 25 patients per day

Qualifications

• Education: Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW). Both options are acceptable.
• Experience: Minimum of 2 years in acute medical social work, including experience in discharge planning.

Additional Information

Strong organizational skills are required to facilitate family meetings and contribute to treatment plan development.

Work Environment

This position is based in an acute care facility, with an emphasis on providing care for adult and geriatric populations. Flexibility to float between nearby facilities may be required.
