Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPL) - Assisted Living Location: Peterborough, NH
Position Type: Direct Hire
Shift: Varied (Combination of 12-hour shifts from 7am to 7pm and 8-hour shifts from 7am to 3pm, rotating every two weeks)
Weekend Requirement: Every other weekend
On-Call Requirement: None
Job Description:
The LVN/LPN is responsible for monitoring the health and well-being of residents and supporting the delivery of care services in an assisted living environment. This position provides clinical oversight, supervises care staff, and ensures high-quality resident care in accordance with state regulations and internal policies.
Key Responsibilities:
• Monitor residents' health status and identify ongoing care needs.
• Ensure appropriate follow-through, assessment and documentation when a resident's status changes.
• Assist the Director of Nursing with assessments at move-in, 30-day, semi-annual and change-in-condition intervals.
• Conduct quarterly care plan updates and complete nursing progress notes.
• Assess and document resident falls, incidents, and changes in condition.
• Maintain timely communication with residents' families and primary care providers.
• Ensure vital signs and weights are obtained monthly.
• Maintain accurate health records following federal and state standards.
• Support the Director of Nursing in maintaining readiness for state surveys.
• Order medications and equipment as needed.
• Report resident concerns or changes promptly to the Director of Nursing.
• Monitor medication administration for accuracy and compliance.
• Process new medication orders and maintain the medication room.
• Uphold infection control practices and confidentiality requirements.
• Safely manage residents' oxygen equipment and monitor usage.
• Attend required meetings, including staff meetings, care plan meetings and other assigned sessions.
Experience and Qualifications:
• Graduate of an accredited nursing program (Required).
• Active state license as a Licensed Practical Nurse (Required).
• Minimum of one year of experience in assisted living, home health, hospital or long-term care (Required).
• Strong knowledge of nursing practices, techniques and assessment skills.
• Ability to manage multiple priorities and maintain strong organizational skills.
• Effective verbal and written communication skills.
• Strong judgment, problem-solving and decision-making abilities.
• Ability to work weekends, evenings and flexible hours as needed.
• Compact license accepted.
