Registered Nurse 3 Consultant Developmental Disabilities

RN 3 Position in Bureau of Nursing and Health Services

This position is in the Bureau of Nursing and Health Services in the Division of Statewide Services. The RN 3 will work under the supervision of the Assistant Director of Nursing and assist with ensuring regulatory oversight of the Preadmission Screening and Resident Review (PASRR) program.

PASRR requirements were first enacted with the federal Nursing Home Reform Act as part of OBRA 1987. This requires all people with a developmental disability and/or mental illness to have a PASRR evaluation prior to nursing home admission to determine if that level of care is required or if their needs can be met in a less restrictive setting. If nursing facility admission is deemed appropriate, it must also be determined if other specialized services are indicated for support of the person.

Job duties will include:

• Using data to monitor the status of people with developmental disabilities in nursing homes, focusing on the need for skilled nursing, specialized services, and compliance with length of stay determinations determined by the Preadmission Screening and Resident Review (PASRR) program.
• In-person and virtual evaluation of people with developmental disabilities in nursing homes to determine the continued appropriateness of the admission.
• Remaining current and knowledgeable of all federal and state regulations and guidance for PASRR and ensuring that OPWDD is compliant.
• Using data to identify people with developmental disabilities in nursing homes that have experienced unreported significant changes in health status that may allow them to return to a community setting.
• Conducting quality assurance reviews of completed PASRR evaluations to ensure people with developmental disabilities are diverted from unnecessary nursing home admissions or are referred for discharge and placement to a community residence.
• Providing technical guidance, oversight, and training to facilitate compliance with State and federal requirements.
• Reviewing and providing targeted instruction, learning opportunities and guidance around medically complex and/or challenging cases presented by PASRR Coordinators, OPWDD Regional Field Office staff, and/or other stakeholders and making determinations and/or referrals as needed.
• Working with the Bureau's Assistant Director of Nursing to develop a program for, and provision of, instructional and learning opportunities for voluntary agencies to meet a person's care needs in the community following hospitalization and skilled nursing facility stay.
• Detailed report writing to document areas for improving and supporting regulations and sub-regulatory guidance.
• Tracking people receiving specialized services in nursing homes to ensure they are being delivered and for required federal reporting.
• Collecting, analyzing and summarizing PASRR data for OPWDD federal reporting.
• Educating hospitals, skilled nursing facilities, provider agencies and care managers, and other providers on PASRR requirements, goals and processes.
• Working with interdisciplinary teams and providers in facilitating hospital and nursing home discharges to community settings.
• Partnering with other Bureau RNs to provide on-site consultation and training to voluntary providers.
• Utilizing record review to identify people who may benefit from specialized services, tracking those in receipt of specialized services in a skilled nursing facility, coordinating with the OPWDD Regional Field Office and creating detailed reports.
• Other duties as assigned to support the division and goals of the agency.

Travel percentage is an estimated average; actual travel time may vary to support the needs of the Bureau. Telecommuting policy will be discussed at interview.

Open Competitive: Possession of a license and current registration as a registered professional nurse in New York State and two years of post-licensure clinical nursing experience working with individuals diagnosed with intellectual and/or developmental disabilities.

Substitution: A master's degree in nursing may substitute for one year of post-licensure clinical nursing experience; and a doctorate in nursing may substitute for two years of post-licensure clinical nursing experience.

Transfer: Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 22 ($82,326), Increased Hiring Salary ($10,852), Geographic Pay Differential ($20,000) is added to the job rate ($104,230).

The PSLF Program forgives the remaining balance on Direct Loans after 120 qualifying monthly payments have been made under a qualifying repayment plan while working full-time for a qualifying employer.
