Responsibilities
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and Interdisciplinary Team.
- Contribute to the creation and execution of an age-appropriate, culturally relevant plan of care, involving the resident, family, and healthcare team to achieve outcomes.
- Recommend revisions to the plan of care based on resident outcomes and coordinate referrals to address all aspects of care; collaborate with the resident/family and Interdisciplinary Team.
- Provide direct and indirect skilled nursing care under the supervision of a Registered Nurse; participate in mentoring and orientation of staff; maintain clinical competency.
- Maintain safety and infection-control practices; adhere to policies; address occupational exposure concerns and bloodborne pathogen precautions.

Requirements
- Active New York LPN license
- BLS certification (American Heart Association or RQI only)
- Minimum 1 year of LPN experience, preferably in a long-term care setting
- Ability to rotate Day/Eve/Night/Weekend shifts (D/E/N/W)
- Demonstrated ability to care for adult and geriatric residents and collaborate with an interdisciplinary team
- This is a travel assignment; eligible to travel and commit to the 27-week contract with holiday-season coverage
