Assignment Details
Contract Length: 27 Weeks

Requirements
- New York LPN Licensure
- BLS Certification (AHA or RQI)
- 1 Year of experience serving in an LPN capacity at a Long-Term Care Facility

Roles & Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents.
- Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and other team members.
- Contribute to the development of culturally relevant care plans that reflect nursing diagnoses and desired outcomes.
- Recommend revisions to care plans based on evaluations of resident outcomes and address the need for referrals.
- Maintain current knowledge and competency in nursing practice and participate in the mentoring/orientation of new staff.
- Ensure compliance with safety protocols while providing care, including potential exposure to blood-borne pathogens.

Compliance / Notes
- All contracts must run through the holiday season and start on a Tuesday or Thursday for full-day orientation.
