LPN - Long-Term Care

Part Time (2 Shifts/3 Shifts Rotational) Weekend and Holiday required

Contract Length: 27 Weeks

Requirements:

• New York LPN Licensure
• BLS Certification (AHA or RQI)
• 1 Year of experience serving in an LPN capacity at a Long-Term Care Facility

Roles & Responsibilities:

• Provide direct and indirect skilled nursing care for assigned residents.
• Collect data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and other team members.
• Contribute to the development of culturally relevant care plans that reflect nursing diagnoses and desired outcomes.
• Recommend revisions to care plans based on evaluations of resident outcomes and address the need for referrals.
• Maintain current knowledge and competency in nursing practice and participate in the mentoring/orientation of new staff.
• Ensure compliance with safety protocols while providing care, including potential exposure to blood-borne pathogens.
