License: Illinois Physical Therapist (PT) License Certifications: BLS (American Heart Association) Must-Have: - Patient transfers - Safe and appropriate body mechanics - Physical therapy diagnoses - Plan of care - Patient education Description: Perform patient transfers with assistance as needed from bed, chair, and floor. Demonstrate safe and appropriate body mechanics to reduce the risk of injury to self, staff, and patients. Treat patients and colleagues with dignity and respect, showing empathy and care, while remaining engaged with those who require services. Develop an appropriate plan of care that integrates goals, treatment, outcomes, and discharge planning. Adjust care based on patient response and available evidence. Educate patients, family members, caregivers, students, and other health care providers using relevant and effective teaching methods. Evaluate clinical findings to determine physical therapy diagnoses and outcomes of care. Perform examinations, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history. Document in accordance with the Clinical Process Policy. Screen patients using procedures to assess the effectiveness of and need for therapy services when appropriate. Utilize support personnel according to legal standards and ethical guidelines. Monitor the care delivered by support personnel and provide regular feedback. Complete annual education as assigned. Evaluate patient impairments related to orthopedic and neurologic conditions. Establish a plan of care based on functional deficits and patient goals. Conduct objective testing for range of motion, strength, and performance of daily functional tasks. Complete daily documentation and billing in therapy EMR. Apply tape for stabilization, facilitation, or inhibition of movement. Administer therapeutic modalities as appropriate, including ultrasound, interferential, and iontophoresis. Communicate with referring providers regarding patient progress and treatment recommendations. Fulfill annual requirements as specified for clinical advancement, which includes supervision of students, completing an approved project, and conducting one in-service training.
