Doctor's Choice Home Care & Hospice Texas is seeking a Registered Nurse Case Manager to administer skilled nursing care in patients' homes under physician orders and the plan of care. The RN Case Manager collaborates with physicians and care teams, provides counseling to patients and families, and contributes to QAPI through ongoing assessments.
