Hospice RN Case Manager — Home Health, M-F, On-Call
Transitions Care
Transitions Care in New Albany, OH is seeking an RN Case Manager to plan, organize and direct hospice care in patients' homes. You will coordinate with the interdisciplinary team to assess needs and adjust the care plan, ensuring timely and compassionate service. You will travel locally, participate in on-call duties, and support families while maintaining professional growth through ongoing education.
