Responsibilities include meeting patients at their bedside to assess eligibility, maintaining a caseload, and fostering teamwork to gather necessary information. The ideal candidate will possess a high school diploma or GED, have basic proficiency in Microsoft Office, and be bilingual in English and Spanish.
