Key Responsibilities

• Conduct comprehensive evaluations of patients recovering from surgery, injury, or chronic illness in a home setting, accurately assessing their physical capabilities and limitations.
• Develop and implement individualized treatment plans based on each patient's specific conditions, goals, and abilities, utilizing a range of therapy techniques, including therapeutic exercise, manual therapy, and modalities.
• Monitor and document treatment outcomes meticulously, including patient progress and responses to therapies, adjusting treatment plans as necessary to ensure optimal recovery.
• Educate patients, caregivers, and family members regarding treatment techniques and modalities, emphasizing the importance of self-care strategies and rehabilitation exercises.
• Recommend and fit patients with assistive devices such as walkers and braces to enhance their mobility and independence.
• Stay abreast of the latest practices in physical therapy through participation in workshops, continuous education, and professional networking.

Patient Population / Client Focus

The Physical Therapist will work closely with a diverse patient population, including children, adults, and elderly individuals facing various rehabilitation challenges. This may include cases related to orthopedic injuries, neurological disorders, chronic pain conditions, post-operative recovery, and geriatric concerns. Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state license to practice as a Physical Therapist in New Jersey.

Compensation and Benefits
