Registered Nurse Case Management

Del Sol Medical Center

Del Sol Medical Center seeks a Registered Nurse Case Management professional to coordinate, evaluate, and advocate for safe, effective patient care transitions. In this full‑service acute care hospital, you will assess clinical, psychosocial, and discharge needs; create and update care plans; and collaborate closely with physicians, nursing staff, social work, and external providers. You will educate patients and families, ensure appropriate utilization of resources, and support regulatory and documentation standards.
