As a nonprofit organization, we partner with individuals to promote healing, housing stability, and long-term wellness through compassionate, equitable support.If you’re passionate about community impact, we invite you to join our team.Pay Rate: $70,000 can go up DOEPurpose: (General description and summary)The Mental Health Clinician (MHC) is a key member of the multidisciplinary team supporting youth in the Rosebud program, a supportive housing initiative for participants ages 16 to 18 who are in the foster care system. This program offers a combination of clinical services (through Medicaid) and wraparound supports (funded by DHS).The MHC is responsible for conducting mental health assessments, providing diagnoses, and developing individualized treatment plans that outline medically necessary services to support treatment and recovery. This role includes delivering a range of services such as skills training, case management, and counseling, primarily in home and community-based settings.In addition to clinical work, the position requires a strong focus on administrative duties, including timely documentation, service authorizations, and compliance with ongoing reporting requirements.Accountabilities:Case Planning and ManagementDevelop relationships with each client on the caseload by meeting for case management, skills training, and/or counseling on an established schedule.Assess the strengths and service needs of each participant.Develop individual service plans in partnership with the participant and chosen supports.Collaborate with DHS caseworkers to create plans for family involvement and integration of family support.Provide clinical recommendations related to treatment, crisis intervention, and service needs to Rosebud team and serve as a point of consultation on clinical issues.Assist participants in connecting to resources and services needed to achieve goals set in individual service and support plan.Monitor service delivery and coordinate care as needed.Develop relationships with providers and stakeholders to facilitate service provision.Ensure scheduling and attendance of prescriber visits for medication management and problem-solving with participants to ensure effective medical services.Provide customer service to New Narrative, Inc.’s stakeholders.CounselingProvide effective behavioral health counseling and psychotherapy to participants in both individual and group settings.Regularly assess and monitor therapeutic progress, ensure that counseling services provided remain medically necessary and consistent with the mental health assessment and individual service plan.Provide limited-duration family style therapy that aligns with participant’s individual goals and provide referrals to long-term family therapy, as needed.Utilize and demonstrate proficiency with Medicaid-approved, evidence-based practicesE.g. motivational interviewing, cognitive behavioral therapy, dialectical behavior therapy, wellness management and recovery, integrated dual diagnosis treatment, etcCrisis InterventionCollaborate with Compliance Specialist to review incoming safety plans and ensure use of safety plan in ongoing services.Take reasonable action to avoid use of crisis services, including consistent use of trauma informed care principles. Collaborate with participant and chosen support on any other proactive steps to minimize use of crisis services.Complete regular risk assessments, safety planning, and crisis alerts as appropriate.Document hospital and ER utilization and collaborate with hospital for discharge planning.Complete incident reports and any other documentation as deemed necessary by contract requirements, agency policy, and state laws.Follow mandatory reporting guidelines for children.Carry emergency phone on a rotating basis.Administration:Complete all required assessments, reauthorization, and funding paperwork.Document all participant contact and actions made on behalf of participants within 72 hours of contact.Complete Rosebud Visual Action Plan and clinical service plans and reviews.Complete monthly updates on progress toward Rosebud program goals, to be sent to ODHS Child Welfare staff.Work with support staff to ensure Rosebud and clinical charts are complete and in compliance with federal, state and county rules.Collaboration with internal Rosebud team including Program Manager, QMHA, Peer Manager, and Peer Support Specialist.Collaboration with external support networks and DHS stakeholdersUtilization ReviewsProvide information as requested by the Compliance Specialist.Complete all documents and observe timelines to ensure that charts are complying.Participating in supervision as needed to review interventions and utilization.Other General Expectations:Attend and positively participate in required meetings.Familiarity with area resources and unique needs of youth, particularly foster youth.Pursue ongoing development of skills and competencies; complete all required trainings to maintain compliance with contract requirements, OARs, and licensing body.Follow all corporate policies, procedures, and protocols.Adhere to any applicable ethics codes.Maintain a QMHP/equivalent credential through MHACBO, OBLPCT, or BLSW.Maintain current Drivers License.Adhere to a regular work schedule as set by direct supervisor.Follow through on all directives and instructions.Maintain security of all corporate interests.Maintain CPR and First Aid Certificate.Positions Reporting: (Listing of job incumbents reporting into this position)NoneEducation/Certificates/Licenses/Registration:Qualified Mental Health Professional with a Masters Degree in social work or related field is required. A minimum of two years post-graduate experience is preferred, experience with working with minors and/or in the foster care system preferred. Experience/training with trauma treatment and trauma informed care preferred. CADC Credentials are preferred.Experience/Qualifications:Bilingual Spanish/English service provision capability preferred. Applicants must possess the skills necessary to work independently. Must have a firm understanding of unique developmental needs of young adults 16-18, individuals with severe and persistent mental illness, co-occurring substance use disorders, psychiatric medications and their use, administrative rules regarding work with minors and other applicable laws and regulations, as well as a firm understanding of acceptable clinical practices and guidelines. Complete understanding of the DSM V and the ability to compose comprehensive mental health assessments and initial service plans.Mental Requirements:Extensive mental activities are required. Examples to include but not limited to the use of educated judgment, communicating thought clearly, maintain calm demeanor in crisis situations, be an empathetic and effective listener, and flexible analytical and interpreting skills.
