Community Care Physicians, a physician-owned multispecialty group in Latham, NY, seeks a Physical Therapist to join our Healthcare & Medical Services team. Provide evidence-based, patient-centered therapy in an outpatient setting, collaborating closely with physicians and clinical staff. Responsibilities include evaluations, individualized treatment planning, therapeutic interventions, and patient education.
