Spring Creek Healthcare Center is seeking a highly motivated and compassionate Physical Therapist to join our dynamic team on a per diem basis. As a Physical Therapist with our state-of-the-art facility, you will have the opportunity to work with a diverse patient population and make a meaningful difference in their lives. As a Physical Therapist, you will work closely with patients to develop and implement individualized treatment plans designed to improve their physical function and mobility. You will assess patients' physical abilities and needs, and design and implement a variety of interventions to address these needs. You will work collaboratively with other members of the healthcare team to ensure that patients receive comprehensive and effective care. You will maintain accurate and up-to-date records of patient care, including treatment plans, progress notes, and discharge summaries. You will participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.

Conduct initial and ongoing assessments of patients' physical abilities and needs. Develop and implement individualized treatment plans, including goals, objectives, and interventions. Provide physical therapy interventions, including exercise programs, therapeutic activities, and education. Monitor and document patient progress, adjusting treatment plans as needed. Collaborate with other healthcare professionals to develop and implement comprehensive care plans. Participate in quality improvement initiatives and contribute to the development of best practices in physical therapy.

Located in the heart of Perth Amboy, New Jersey, Spring Creek Healthcare Center is a 179-bed nursing home with a strong reputation for delivering high-quality care and service to our patients. Our center is committed to providing a welcoming and inclusive environment for patients from diverse backgrounds, including our dedicated Hispanic Program, Mi Hogar. We are expanding our post-acute programming in collaboration with area specialists and undergoing full-scale renovations to enhance our facilities and services.
