SPHINX HOME HEALTH CARE

Job Description

Provide skilled home health nursing care to geriatric patients across multiple communities. This role delivers skilled nursing visits and chronic disease management under the clinical supervision of a Registered Nurse case manager, supporting patients and families through compassionate care, education, and coordinated services.

Responsibilities

Conduct patient assessments and monitor vital signs during home visits.
Administer medications and provide IV care in accordance with clinical orders.
Deliver wound care and support infection control practices.
Develop and implement individualized care plans for geriatric patients.
Provide patient and family education, caregiver communication, and family support.
Coordinate care with the RN case manager, physicians, caregivers, and interdisciplinary teams.
Complete daily electronic charting and clinical documentation in AXXESS EMR.
Participate in interdisciplinary care conferences and manage physician orders.

Required Qualifications

Five or more years of experience in home health nursing.
Completion of an accredited Licensed Practical Nursing program.
Active Licensed Practical Nurse (LPN) license.
Basic Life Support (BLS) certification.
Proficiency in medication administration, wound care, and patient assessment.
Knowledge of electronic health records, infection control, and care planning.
Strong clinical documentation, patient education, and communication skills.
