This role involves direct patient care and collaboration within a dynamic healthcare environment. The selected nurse will cover counties including Okaloosa and Walton, providing critical case management services to high-risk members.

Key responsibilities of the RN – Case Management:

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement individualized care plans.
• Address and reassess cost-efficient and appropriate levels of care.
• Ensure non-medical support for treatment plan compliance.
• Coordinate with healthcare professionals in patient transitions.
• Engage specialty resources as needed for optimal outcomes.
• Maintain detailed records of management outcomes.

• Act as a patient advocate by protecting privacy and confidentiality.

Requirements of the RN – Case Management:

• Associates or Bachelor Degree in Nursing or related field.
• Current, unrestricted state Registered Nurse license (Florida or compact license accepted).
• Experience in analyzing complex medical information and making decisions.
• Competence in Home Care Home Base/Pointcare and OASIS collection.
