Licensed Social Worker Short Term Coverage

Role Overview Harbor House Nursing & Rehabilitation is seeking a Licensed Social Worker (LSW) to join our exceptional team on a temporary basis through December 2026. This coverage role is anticipated to begin in Spring 2026. Requirements Must possess a minimum of a B.A. in Social Work, MSW preferred Active Social Work Licensure in Massachusetts, in good standing LSW, LCSW acceptable; LICSW preferred 1+ years skilled nursing facility experience, preferred A strong desire to support and guide residents and their families through their later years with compassion and dignity Job Duties Complies social history data Participates in assessments, development and evolution of residents care plan Responsible for planning and leadership interdisciplinary care planning weekly meetings Documents timely progress notes according to state and federal regulations Coordinates the discharge planning for residents Able to interpret and implement established social service policies, procedures and regulations Coordinates services of community agencies for continued delivery of care upon discharge We are proud to be an equal opportunity workplace.
