At Altais, we support one another, adapt quickly, and work with integrity as we build a better experience for physicians and their patients.Our NP or PA will perform comprehensive health assessment which include but not limited to the following:* Obtain history and perform physical examination* Complete review of medicines and related instructions* Review home safety and psychosocial needs* Fulfill documentation needs for STAR and HCC measures* Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions* Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies* Maintain patient safety and confidentiality* Universal precautions utilized in all patient care* Prescribe/order procedures, treatments and medications within scope of practice* Develop high level of trust and intimacy with patients and families**The Skills, Experience & Education You Bring*** CA Licensed Nurse Practitioner or Physician Assistant* *1+ Years of Experience with Primary Care*The Base Salary for this position is **$125,000 - $145,000/yr** In addition, we provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.* Excellent medical, vision, and dental coverage* 401k savings plan with a company match* Flexible time off and 9 Paid Holidays**You Share Our Mission & Values:****Compassion**We act with empathy and a deep respect for the challenges faced by physicians and their patients.
