Patient Care Coordinator

Summary: Responsible for assessing patient needs and linking patients to community agencies and resources as dictated by patient need.

Conducts an initial assessment and gathers data relevant to patient needs.

Assess patient needs and capabilities in the context of specific referral requests made by the primary clinician.

Gathers and documents all necessary information needed for making patient referral, e.g., proof of patient income, current medication dosages, etc.

Coordinates services and resources to meet patients ancillary needs.

Experience: 1-3 years

Credentials: Bachelor's Degree in Human Services or related field required. Valid MD Driver's License required and clearance by the LBH Fleet Safety Department to operate vehicles.

Pay Range: $20.06 to $30.09 Based On Experience
