Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant). Job Overview As an in‑home clinician, you will conduct health evaluations in the comfort of patients’ homes, providing a comprehensive, personalized assessment and connecting patients to the right care. The visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and, when ordered, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad and an intuitive clinical workflow, and you are not required to prescribe medicine, order lab tests, or alter existing treatment regimens. Responsibilities Provide in‑home health evaluations to patients and families. Conduct medication and medical history reviews, and perform physical assessments. Utilize iPad technology and clinical workflow tools to deliver care. Travel locally and within licensed states as required and conduct virtual visits as needed. Offer diagnostic and preventive services including spirometry, diabetic eye exams, and bone density screening. Build trusted relationships with patients to improve health outcomes. Qualifications Active, unrestricted license(s) in the coverage area(s) (Nurse Practitioner or Physician Assistant). Board certification (required). Multi‑state licenses (preferred). Pay The typical pay range for this role is $78,331.00 – $168,714.00, reflecting the base hourly rate or base annual full‑time salary. The actual offer will depend on experience, education, geography and other relevant factors.
