Director of Case Management & Social Work Services

St. Mary's General Hospital in Passaic, NJ, seeks a Director of Case Management to lead a team of case managers, social workers and discharge planners. The role focuses on developing staff, systems, and processes to deliver a comprehensive program aligned with the hospital's mission and patient needs. The ideal candidate will have an LCSW or licensed clinician, at least five years post-graduate Social Work experience, and comfort with data analysis and utilization review tools; CCM certification #J-18808-Ljbffr Prime Healthcare
