Job Description
The Case Manager will work independently in a support role primarily managing social services cases. This position involves assessing client needs, providing resource referrals, and maintaining detailed case documentation using general office software and the HMIS database system. The Case Manager plays a critical role in offering follow-up support to ensure clients receive the necessary assistance with no travel required.
Responsibilities
- Conduct thorough client assessments to identify needs and goals
- Refer clients to appropriate community resources and services
- Maintain accurate and up-to-date case documentation using HMIS and office software
- Provide consistent follow-up support to monitor client progress
Required Qualifications
- Minimum 1 year experience in case management
Preferred Qualifications
- Bachelor's degree in Social Work or related field
- Experience with client needs assessment
- Strong documentation and reporting skills
- Excellent communication, empathy, and problem-solving abilities
