Case Manager – Home Health (Part‐Time)

Location: Clifton, NJ and surrrounding areas
Agency: Care With Passion Home Health Care Services, LLC

Care With Passion Home Health Care Services is seeking a highly organized, dedicated Case Manager to oversee client care, conduct assessments, complete supervisory visits, and ensure the delivery of high‐quality home care services. This role is essential to maintaining compliance, client satisfaction, and caregiver support.

Responsibilities

• Assess patient needs and develop individualized care plans
• Coordinate home care services and monitor patient progress
• Communicate effectively with patients, families, and healthcare providers
• Evaluate home safety and recommend necessary modifications
• Ensure compliance with patient privacy regulations such as HIPAA
• Problem-solve issues related to patient care and service delivery
• Manage time effectively to meet patient and organizational needs
• Build strong relationships with patients and caregivers
• Monitor changes in client condition and update care plans as needed
• Coordinate caregiver assignments based on client needs and preferences
• Communicate regularly with clients, families, caregivers, and healthcare providers
• Respond to incidents, concerns, and service issues promptly
• Maintain accurate documentation in EMR/CRM systems
• Ensure compliance with NJ DOH regulations and agency policies
• Support caregiver coaching, training, and performance feedback
• Promote client satisfaction, retention, and service excellence

Required Qualifications

• High school diploma required; Associate’s or Bachelor’s degree preferred
• 1–2 years of experience in home care, healthcare, human services, or case management
• Reliable transportation for field travel
• Ability to conduct home visits and supervisory visits
