Hospice RN Case Manager - Day Shift (Travel 50%)

VIA Health Partners

VIA Health Partners is seeking a Full Time Nurse for the Day Shift in Charlotte, NC, focusing on patient care coordination and education. Candidates must hold a nursing diploma or associate degree, with a preference for a Bachelor’s Degree, and a current RN license. The successful applicant will join a dedicated healthcare team and will be responsible for assessing patients, developing care plans, and maintaining accurate documentation.
