Your Role and Responsibilities

• Performing thorough patient evaluations, scheduling necessary diagnostic tests, and crafting customized care plans
• Diagnosing and managing acute, episodic, or chronic conditions in a collaborative environment with physicians or independently (based on your qualifications)
• Providing education to patients and families regarding preventive measures, wellness strategies, and chronic disease management
• Prescribing medication and therapies in accordance with your scope of practice
• Engaging in follow-up care and chronic disease management
• Working with interdisciplinary teams to ensure comprehensive and coordinated care
• Maintaining accurate and detailed records within electronic medical records (EMR)

Academic Credentials and Qualifications

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Graduation from a recognized Nurse Practitioner program

Licensing and Certification Criteria

• Active NP licensure in the state of New York
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or relevant equivalent
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or willingness to obtain within 90 days)
