Home Health Licensed Practical Nurse

Role Overview: Provide personalized, one-on-one skilled nursing care in patients' homes, supporting recovery and independence. Collaborate with healthcare teams, implement care plans, and educate patients and families on disease management and self-care techniques. Key Responsibilities Deliver onsite nursing care under RN supervision. Coordinate with patients, families, and healthcare team to develop and execute care plans. Maintain accurate documentation of patient visits, interventions, and outcomes. Communicate promptly with physicians and team members regarding patient needs and status changes. Participate in care coordination and discharge planning activities. Qualifications & Skills Valid nursing license in the practicing state. Current CPR certification. At least two years of clinical LPN/LVN experience. Reliable transportation, valid driver's license, and auto insurance. Preferred experience in Home Health or Hospice settings. Compensation & Benefits Competitive pay per visit/unit, ranging from $37.00 to $52.00, with full-time annual pay approximately $58,300 to $80,300.
