Clinical Pharmacist

We're seeking an experienced Clinical Pharmacist for a hospital assignment in Portland, Oregon. If you're looking for a fast-paced inpatient environment where you can utilize your clinical expertise and collaborate with an interdisciplinary care team, this is an outstanding opportunity.
What You'll Do- Verify and review medication orders for accuracy and appropriateness
- Provide clinical pharmacy services to hospitalized patients
- Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy
- Perform pharmacokinetic dosing and therapeutic drug monitoring
- Participate in clinical interventions and medication safety initiatives
- Utilize Cerner and Pyxis for daily pharmacy operations
- Provide drug information and clinical recommendations to the care team
- Active Oregon Pharmacist license
- Strong recent inpatient clinical hospital pharmacy experience
- Experience with order verification and clinical pharmacy services
- Experience with Cerner and Pyxis preferred
- Excellent communication and critical thinking skills
