Clinical Pharmacist Relief - Emergency Department Location: US-OR-Portland Requisition ID: 2026-39702 Position Category: Pharmacy Job Type: AFSCME union represented Position Type: Relief/Flex/Resource Posting Department: Inpatient Pharmacy Posting Salary Range: $77.20 - $106.00 per hour (offer based on experience, education and internal equity) Posting FTE: 0.00 Posting Schedule: Monday - Sunday Posting Hours: 6:00am-3:00pm; 2:30pm-11:00pm and 9:00am-7:30am; Variable - mornings, afternoons and overnights Department Overview The pharmacist will experience competing demands from nurses, physicians, pharmacy staff, and other healthcare professionals as they attempt to influence clinical decision‑making, clinical policies, and practices and workflow. They must handle intense situations on a daily basis, with frequent interruptions that may lead to mental fatigue or stress. The pharmacist is responsible for the safe and proper distribution and use of medications in inpatients and outpatients ranging in age from neonates to over 65. The role includes medication reconciliation, discharge counseling, coordination of care from admission to discharge, and collaboration with interdisciplinary teams. Additional responsibilities include teaching and precepting students, interns, and residents. Responsibilities Identify, resolve, and document complex therapeutic problems, demonstrating advanced knowledge. Interpret and evaluate physician orders and prescriptions for accuracy, completeness, and appropriateness; document activities related to decision or distribution issues. Assist in medication reconciliation and patient education regarding home medication regimens; coordinate discharge medication plans. Monitor therapeutic responses and adverse effects of medications to guide therapy; report adverse drug reactions, defects, and errors. Respond accurately to drug information requests from clinicians and patients; review literature and apply findings to patient care. Other duties as assigned. Required Qualifications Current pharmacist license issued by the Oregon State Board of Pharmacy. OHSU may require specialization training or experience at time of recruitment. Job Related Knowledge, Skills, and Abilities Inpatient hospital pharmacy practice across a wide range of patient types. Adherence to pharmaceutical care principles and safety. Ability to modify drug therapy for age and disease. Evaluation of medical literature. Independent work without minimal supervision. Task management amid frequent interruptions. Effective written and oral communication, including presentations. Professional team collaboration and project management. Prioritization for emergent situations. Cooperative work with health‑system and pharmacy staff. Attention to detail and systematic problem solving. Judgment in enforcing guidelines, making critical decisions, and performing key computer skills. Preferred Qualifications PGY-2 specialty residency completion. BLS/ACLS certification, vaccination certification, teaching certificate. Experience in outpatient pharmacy rotations, transitions of care, discharge education, surgical or internal medicine services, pain management, medication reconciliation.
