Home Health Licensed Practical Nurse (LPN)

Per Diem or Full-Time | Phoenix Valley, Arizona

Billet Health is looking for compassionate and dedicated Licensed Practical Nurses (LPNs) who have the passion and heart for providing exceptional patient care.

Our Home Health team provides skilled care to geriatric patients in the comfort of their own homes and residential facilities. As an LPN, you will work closely with the RN Case Manager and Director of Nursing to assess patient needs, provide quality care, and help ensure each patient receives the attention and support they deserve.

FLEXIBLE SCHEDULING - YOU SET THE PACE

At Billet Health, we believe our nurses should have flexibility and control over their schedules.

As patients come onto service, our staffing team will contact you when visits are available in your service area.

You choose how much you want to work.

Whether you want to:

• See several patients each day
• Work a few days per week
• Pick up occasional visits
• Build a full-time schedule

-you have the flexibility to determine the workload that works best for you.

We do not dictate a specific time for your patient visits. Once a visit is assigned, you will coordinate directly with the patient or caregiver to arrange a visit time that works for both of you.

Typical visits are approximately 45-60 minutes, depending on the patient's needs and the services provided.

YOUR ROLE

As a Home Health LPN with Billet Health, you may be responsible for:

• Providing skilled nursing care in patients' homes or residential facilities
• Assessing patient needs and reporting changes in condition
• Following the established plan of care
• Working closely with the RN Case Manager and Director of Nursing
• Providing patient and caregiver education
• Monitoring and documenting patient progress
• Communicating with physicians and members of the interdisciplinary care team
• Promoting patient safety, comfort, and independence
• Completing accurate and timely clinical documentation

QUALIFICATIONS

To provide Home Health services with Billet Health, you must have:

• Active LPN license in the state of Arizona
• Valid driver's license
• Reliable transportation
• Current auto insurance
• CPR or BLS certification
• Valid TB documentation
  • Skin test within the last year, or
  • Chest X-ray within the last five years

COMPENSATION

Starting at $45.00 per hour

Pay rates and visit compensation are established based on the service territory and applicable patient assignments. Additional compensation may be available for assignments outside the standard service area.

SERVICE AREA

Phoenix Valley, Arizona

We are seeking LPNs throughout the West Valley, Central Phoenix, North Valley, and East Valley, including surrounding communities based on patient needs.
