Certified Nurse Aide - Skilled Nursing Facility

Overview Provide nursing care to residents age 16 and over who require skilled long‑term and short‑term care, including chronically ill, geriatric, and infirm residents. The primary purpose is to render routine daily nursing and restorative care and services in accordance with the resident’s assessment and care plan and under the direction of a professional nurse. Reports to the Nursing Supervisor or Unit Director. Sign on Bonuses up to $5,000! Duties and Responsibilities Follow work assignments and/or work schedules to complete tasks. Perform all tasks in accordance with established policy and procedure and as instructed by the Professional Nurse. Receive the nursing report upon reporting in and give reports to Charge Nurse before ending shift duty hours. Wear photo identification card at all times. Record all entries on flow sheets, nutrition sheets, elimination record, activity record, rehabilitation record, and complete all documentation in electronic form as instructed. Report all changes in the resident’s condition to the RN or LPN Charge Nurse as soon as practical. Report all accidents and incidents observed on the shift as they occur, providing a statement. Participate in performance assessment and improvement activities for continued quality improvement of resident care. Perform routine resident care, including bathing, mouth care, hair care, nail care, hygiene, toileting assistance, feeding, ambulation assistance, and promote resident comfort (positioning, lifting, and transfers). Answer call lights promptly and assist with emotional needs. Observe and report presence of pressure areas and skin breakdowns to prevent decubitus ulcers. Assist with post‑mortem care. Takes and records weights, temperature, pulse, respiration, and blood pressure as directed. Promote adequate resident nutrition by participating with meal service and feeding residents. Assist residents with in‑between snacks and nourishment as assigned. Collect and transport specimens as directed. Assist in the care of the patient in isolation. Perform prescribed rehabilitation nursing exercises, range of motion, and use of adaptive equipment. Fold and deliver resident’s personal clothing. Care Plan and Assessment Functions Assist with carrying out the resident assessment. Participate in care plan conferences to develop and review resident care plans. Provide nursing care according to the established care plan. Recommend changes in the resident’s care plan and goals based on observations and assessments. Review resident’s closet and care plan, informing the nurse of any changes in patient condition. Safety and Sanitation Operate all equipment safely after receiving in‑service training. Report defective equipment immediately to Unit Director or Charge Nurse; label and inform Unit Clerk to order repair. Clean resident equipment such as commode chairs, wheelchairs, shower chairs, and geriatric chairs. Follow policies and procedures related to standard precautions. Keep resident rooms, dresser drawers, closet, utility rooms, shower/tub rooms, and corridors neat and in order. Follow New York State requirements for PPD testing. Follow emergency protocols related to fire and disaster procedures. Staff Development Attend continuing educational programs to enhance knowledge of residents requiring skilled nursing and meet CNA certification requirements. Participate in DNV and OSHA mandated in‑services annually. Promote and foster the mission and philosophy of the facility during job duties and community activities. Support and participate in continuous quality improvement endeavors. Perform other related duties as required. Resident Rights Functions Maintain confidentiality of all resident care information. Respect and uphold residents’ rights to independent expression, decision, personal freedom, dignity, and freedom from abuse or social isolation. Provide privacy for residents, knocking before entering and closing doors or curtains before procedures. Report resident complaints or grievances to Charge Nurse as soon as possible. Other Attend all annual mandatory educational programs as required. Interact with residents in a friendly and caring manner, being attentive to both psychological and physical needs, and maintain secure and pleasant experiences for residents. Recognize that this job description lists typical duties and may not include all duties which may be assigned. Education High school graduate preferred. Experience Previous experience or job knowledge is required. Nursing aide certification is required. If hired without certification, must complete the requirements for nurse aide certification within 120 days. CPR Requirements Heartsaver CPR required within 90 days of hire date. Basic Life Support (BLS) required within 90 days of hire date. Physical Demands Good physical and mental health is required because long periods of standing and walking are involved. Stretching and lifting are required in the care of patients, equipment, and the physical environment. Manual dexterity is necessary for handling instruments and equipment. Visual and aural acuity are required to make adequate observations of resident condition. Special Requirements Demonstrate understanding, patience, and tact when dealing with residents and coworkers. Abide by and support hospital policies and standards. Trustworthiness in safeguarding confidential information. Willingness to work hours that best meet the needs of a specific service. Maintain a neat and professional appearance at all times. Limited access to the medical record; includes charting on the flow sheet, ADL, and graphics. Other Requirements The certified nurse aide must be listed as current in the New York State Nurse Aide Registry, with their certification status verified and no findings of abuse or misconduct. The printed verification must be filed in the personnel record. Exposure Category Category I. Tasks involve exposure to blood, body fluids, or tissues. #J-18808-Ljbffr
