In your role, you will evaluate patients, whether in inpatient or outpatient settings, and craft individualized treatment plans tailored to meet their unique needs. By administering targeted physical therapy interventions focusing on mobility, strength, balance, and function, you will collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to maximize outcomes. You will also serve as an educator for patients and their families, guiding them through therapeutic goals, discharge planning, and home exercises, while meticulously documenting evaluations and progress in compliance with facility regulations. Additionally, you will participate in interdisciplinary conferences and care team rounds to ensure seamless and coordinated care.

To qualify, you should possess a Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution, along with a state license to practice as a Physical Therapist. While CPR certification is a plus, specialized credentials in areas like neurology or geriatrics will be considered an asset but are not mandatory. Previous experience is highly desirable, along with an ability to address complex medical and mobility issues.
