Description

• Assess patients in both inpatient and outpatient settings and create tailored rehabilitation plans
• Deliver therapy interventions focused on enhancing mobility, strength, balance, and overall function
• Work closely with nurses, physicians, case managers, and various rehabilitation specialists to maximize patient outcomes
• Guide patients and their families through therapeutic objectives, discharge planning, and home exercise recommendations
• Maintain detailed records of evaluations, treatments, and patient progress following facility and regulatory standards
• Engage in interdisciplinary rounds and conferences to promote collaborative care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
