Nurse Care Coordinator Manager (RN)Clarity Pediatrics is reimagining pediatric chronic care, starting with ADHD. After successfully launching our virtual clinic for ADHD and anxiety, we are expanding our pediatric care platform to address other common chronic conditions and are focused right now on Obesity and Overweight management. Our clinical model is tech-enabled, backed by the best science, and designed to deliver exceptional health outcomes that transform a child's trajectory.The Nurse Care Coordinator Manager (RN) is an integral member of Clarity Pediatrics' interdisciplinary care team, collaborating with physicians, advanced practice providers, psychologists, and therapists. Under the direction of clinical leadership, the Nurse Care Coordinator lead ensures seamless care delivery for children with chronic conditions by coordinating clinical appointments, providing patient and family education, managing medication authorizations, and serving as a primary point of contact between families, providers, and pharmacies. This role will also be the supervisor for the team of nurses, medical assistants, and community health workers. Note this is a fully remote position.Key Responsibilities:Leadership Responsibilities Include:Staff Supervision & DevelopmentRecruit, hire, train, and onboard new team membersProvide direct supervision and performance management for all assigned staffConduct regular 1-on-1 meetings, performance reviews, and coachingIdentify professional development needs and facilitate training and continuing educationAddress performance issues promptly and constructively; document as appropriatePrepare and deliver staff schedules and manage coverageOperational ManagementMaintain compliance with healthcare regulations (HIPAA, state nursing boards, etc.)Ensure timely and accurate documentation and record-keepingParticipate in quality improvement and process optimization initiativesTrack and report key performance indicators and metricsCoordinate with other departments to ensure seamless patient care transitionsMaintain appropriate staffing ratios and coverageCommunication & CollaborationCommunicate effectively with patients, families, and interdisciplinary team membersParticipate in team meetings and clinical huddlesServe as liaison between clinical staff and managementReport to leadership on team performance, concerns, and successesCollaborate with community partners and external resourcesAlso assist with coverage and support of Care Coordination Team duties, including:Care Coordination & SchedulingCoordinate, schedule, and reschedule clinical appointments across the care teamManage referrals to specialist and external clinical servicesProactively follow up with families to reduce gaps in care and no-show ratesCommunicationProvide clinical updates to referring physiciansCoordinate lab work and documentation requirements for treatment planning with referring providersSupport the providers for patients requiring escalation of careProvide evidence-based education to families regarding chronic condition managementExplain medication regimens, treatment plans, and care goals in accessible languageRespond to patient and family inquiries via messaging and phone in a timely mannerMedication Management & Pharmacy CoordinationInitiate and manage medication prior authorization requests with insurance payersCommunicate with pharmacies to resolve dispensing issues and coordinate refillsTrack authorization status and escalate delays to the appropriate clinical team memberMaintain accurate medication records and document all pharmacy communicationsQualifications:Licensure: Active California Registered Nurse (RN) license in good standing (Bachelor's, or Master's degree in Nursing accepted)Experience: Minimum 10 years of nursing experienceLeadership: Experience leading a team of clinical staffTech-Savvy: Demonstrated proficiency with multiple technology platforms and electronic health record (EHR) systemsCommunication: Strong organizational, communication, and interpersonal skillsVirtual Environment: Ability to manage competing priorities and work collaboratively in a remote, virtual environmentCultural Responsiveness: Comfort working with diverse, medically and socially complex pediatric populationsStartup Mindset: Excitement about joining a fast-growing startup and iterating on new care models.Internet Access: Reliable internet connection and a dedicated workspace suitable for remote workExperience in pediatric, chronic care, or care coordination settingsFamiliarity with prior authorization processes and payer requirementsExperience working in underserved or urban environmentsLogistics:Full-time positionRegular work hours of M-F from 8 AM - 5 PM PT + on-call coverage for escalation of care by Care Coordinator Team$100,000 - $120,000/year with medical and ancillary benefitsRemote-friendly culture
