RN Care Manager

RN Care Manager – Evanston, IL Hourly Pay Range: $40.45 - $62.70, determined by expertise and years of experience. Position highlights: Full-time, 40 hours/week Monday-Friday, 8:30 a - 5:00 p, rotating every 4-6 weeks One major winter and summer holiday coverage required What you will need Bachelor’s Degree in Nursing (required) or Master’s Degree in Nursing (preferred) 2+ years of clinical nursing experience, preferably in an acute care or hospital setting Current Registered Nurse license in Illinois (IDFPR) Case Manager Certification (CCM) or related field (preferred) CPR/BLS certification (AHA) What you will do Conduct thorough assessments of patients’ medical, social, and psychological needs Collaborate with healthcare providers to develop comprehensive care plans Identify and address barriers to care and coordinate services accordingly Serve as a liaison between patients, families, providers, and community resources Monitor and manage patient progress through the healthcare continuum Coordinate referrals, home healthcare services, and appointments Oversee transitions of care, including admissions and discharges Provide patients and families with information about diagnoses, treatment options, and support resources Educate patients on self-management strategies Maintain accurate electronic health records Prepare reports on patient progress and outcomes Collaborate with interdisciplinary team to improve care processes Participate in quality improvement initiatives Ensure compliance with HIPAA and other regulations Benefits (for full and part-time positions) Premium pay for eligible employees Career pathways for professional growth Medical, dental, and vision options Tuition reimbursement Free parking at designated locations Wellness program Savings plan Health savings account options Retirement plan with company match Paid time off Community involvement opportunities EEO Statement Endeavor Health is an Equal Opportunity Employer.
