Hybrid Home Health RN Care Manager
Optum
Caretenders Home Health, part of the Optum family, is seeking a Patient Care Manager RN Hybrid to supervise and coordinate clinical services for home health patients under physician orders. You will lead an interdisciplinary team to ensure continuity and high-quality care in alignment with the plan of care and state and federal regulations. Primary responsibilities include directing home health aides and LPNs, performing comprehensive assessments (OASIS), coordinating referrals and start of #J-18808-Ljbffr Optum
