Discharge Planning & Social Services Assistant

Restore Health Rehabilitation Center seeks a Social Services Assistant to support discharge planning under the Social Services Director. You will coordinate patient support from admission through discharge, completing initial assessments and interviews. You will inform patients and families about Medicare, Medicaid, and available financial assistance, and collaborate with Case Management to ensure smooth transitions. #J-18808-Ljbffr Restore Health Rehabilitation Center
