Department/Program: Residential Recovery Services Reports To: Clinical Director Supervisory Responsibilities None Schedule: Friday-Monday 7:30am - 3:30pm Bay Cove Human Services’ mission is to partner with people to overcome challenges and realize personal potential. Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and southeastern Massachusetts. Job Summary: The Case Manager provides support, counseling, and community referrals to individuals with substance use and co-occurring disorder, including facilitating individual and group counseling sessions and providing aftercare case management planning services. The Case Manager contributes to Bay Cove’s mission by directly partnering with individuals and coordinating the services necessary for their recovery journey. This is a non-exempt position. Essential Functions of Position Orient each individual newly admitted to the program, inform them of their rights, and familiarize them with the community guidelines, procedures, activities, policies, and philosophy of the program, including program requirements for participation, disciplinary action, discharge, and grievance procedures as well as providing informed consent and obtaining signature on consent forms. Under the guidance of the Clinical Director, engage residents in information gathering and decision making to inform the development of the psychosocial assessment, the setting of treatment goals, determination of service needs, and aftercare planning. Coordinate services with external treatment providers, outpatient counselors/therapists, opioid treatment, and other partners such as primary care, DCF, Courts, and assist residents to coordinate and attend essential appointments as needed during their stay. Develop aftercare plans that are in line with resident needs, interests, and eligibility, make referrals, facilitate interviews, secure placement, and coordinate a warm transition to aftercare services. Provide individual and group education and recovery support appropriate to resident medical and mental status during detox/withdrawal and within the limitation of assessed competency and qualifications. Document all services provided and resident response to those services in the Electronic Health Record. Work collaboratively with the multidisciplinary team to identify changes in resident wellbeing, progress made, and development of new needs, and needed changes to the treatment plan and document the results of these discussions in a Multidisciplinary Service Note. Provide crisis prevention and intervention, escalating to the appropriate party when assistance is needed (supervisor, AMCI Team, etc.). Coordinate each resident's discharge in concert with nursing and aftercare providers and complete all required discharge documentation. Conduct and document post-discharge follow-up for clients within 30 days of discharge. Other job-related activities as assigned. All BSAS licensed programs must have at least one staff member designated to serve as coordinator for the following topics. If assigned, both supervisor and employee must initial to acknowledge the assignment: Serve as the program’s Access/CLAS Coordinator. Support the team in the evaluation, improvement planning, and implementation of efforts to ensure equitable access to services as required by 105 CMR 164.040(A)(11). Attend CLAS training. Serve as the program’s HIV/AIDS/HEP C Coordinator. Oversee resident and staff education, risk assessment, access to counseling and testing, and standards of care. Attend PACNET meetings and relevant Bureau of Infectious Disease meetings, conferences and trainings. Serve as the program’s Tobacco Education Coordinator (TEC). Oversees implementation of BSAS guidelines for integrating tobacco assessment, education, and treatment. Attend TEC meetings and trainings. Serve as the program’s Human Rights Coordinator (HRC). Provides staff and residents with training on resident rights and grievance processes and serves as liaison to the agency’s human rights department. Requirements for the position High school diploma or equivalent required. Bachelor's degree in social work or related field preferred. For those without a bachelor’s degree, equivalent education, training, and/or professional or personal experience will be considered. Minimum of one (1) year experience working in substance use disorder/behavioral health treatment settings required. Demonstrated knowledge of health care, recovery, substance use and co-occurring disorders, DSM Criteria, and terminology is preferred. Demonstrated ability to speak clearly and persuasively in a variety of situations required. Intermediate knowledge of word processing and other computer functions using Windows, Word, and Google Email and Apps required. Certification or eligibility for certification in substance abuse counseling or willingness to pursue certification is welcome, encouraged, and will be supported. An ability to learn and understand federal regulations pertaining to confidentiality of substance abuse treatment is required. Physical Requirements Ability to enter and navigate various physical environments, including multi-story buildings without the use of an elevator. Ability to use a computer, tablet, and mobile phone for data entry, communication, and record-keeping. Ability to remain in a stationary position (e.g., sitting at a desk or in meetings) for extended periods while completing documentation and administrative tasks. Frequent movement throughout the program to conduct wellness/safety checks, check the physical environment, and move supplies. Frequent and clear communication with individuals, colleagues, and partners, requiring the ability to exchange accurate information in person, by phone, and in writing. Occasional lifting or carrying of program materials, supplies, or small equipment (up to 20 pounds). Ability to provide Cardiopulmonary Resuscitation (CPR) and overdose rescue (naloxone administration) if needed.
