CCBC provides inpatient, outpatient, and urgent care services to adults and children overcoming depression, anxiety, mood disorders, substance abuse, and many other conditions hindering personal fulfillment. Join Community Counseling of Bristol County in launching our historic Community Behavioral Health Center (CBHC) program. Description of Position We are seeking a full-time Care Coordinator to join our CBHC team in providing assistance to our clients with mental health struggles. The ideal candidate is interested in providing a compassionate and full continuum of care to clients under the umbrella of the CBHC. You will be part of a multidisciplinary team addressing the social, emotional, psychiatric, and medical needs of the clients we serve, including clients across the lifespan with a variety of presentations, including mental health and substance use concerns. Responsibilities Coordinating medical and behavioral health care and making referrals Working to identify, mitigate, and lift social and health inequities and barriers and advocate for the individual's access to services. Maintaining regular contact with client, provider, family, social service providers, and natural community supports to ensure treatment plans are implemented. Providing education and developing safety plans with clients and families. Requirements Must hold a Bachelor's degree in a mental health field. Must have at least three (3) years of human services related experience, preferably with individuals across the lifespan with serious mental illness and substance abuse. Must possess good organizational skills and effective written, verbal, and computer skills.
