Care Coordinator

Job Title: Coordinator Location: West Hills, CA 91304 Position Overview The Care Coordinator supports care coordination and administrative processes from patient admission through discharge. This role ensures timely and accurate processing of prior authorizations, discharge planning activities, and patient documentation while partnering closely with clinical teams and skilled nursing facilities (SNFs). The ideal candidate is detail-oriented, organized, and experienced in healthcare operations, with strong knowledge of medical terminology and insurance processes. Key Responsibilities Process and submit prior authorization requests, ensuring timely approvals Support discharge planning by coordinating patient transitions and follow-up care Verify admissions from the prior day and confirm accuracy of patient records Request and track clinical documentation to support authorization and care coordination workflows Open and manage authorizations, including status follow-ups Review and interpret Explanation of Benefits (EOBs) Assist in completing the full admission process from intake through final documentation Communicate and collaborate with Case Managers, clinical staff, and external facilities (including SNFs) Facilitate transfer of patient information to appropriate care settings Perform high-volume data entry with accuracy using internal systems and Excel Maintain organized records and ensure timely completion of all clerical functions Required Qualifications High-volume data entry experience, including Excel Experience with prior authorization processing and discharge planning Strong understanding of medical terminology Ability to review and understand Explanation of Benefits (EOBs) Excellent organizational skills and attention to detail Strong communication and customer service skills Ability to manage multiple tasks in a fast-paced healthcare environment Work Environment On-site role in a fast-paced healthcare setting Team-oriented environment working closely with clinical and administrative staff Position supports backlog reduction and operational efficiency initiatives Why Join This Opportunity Opportunity to gain hands‑on experience in care coordination and managed care processes Collaborative environment working alongside experienced clinical teams High‑impact role supporting patient care transitions and outcomes Pay And Benefits Pay range: $25.00 - $25.00/hr.
