About the Role:
As a Physical Therapist with Clarion Nursing and Rehab, you will be responsible for providing high-quality physical therapy services to our residents, including:
Evaluating and treating residents with a wide range of physical disabilities, injuries, and illnesses
Developing and implementing individualized treatment plans to achieve optimal functional outcomes
Collaborating with interdisciplinary teams to ensure coordinated care and effective communication
Maintaining accurate and detailed records of treatment plans, progress, and outcomes
Responsibilities:
Provide Physical Therapy services to residents, including evaluations, assessments, and treatments
Develop and implement individualized treatment plans to achieve optimal functional outcomes
Collaborate with other healthcare professionals to ensure comprehensive care
Maintain accurate and detailed records of treatment plans, progress, and outcomes
Participate in quality improvement initiatives to enhance patient care and outcomes
