About the Role:

As a Physical Therapist with Clarion Nursing and Rehab, you will be responsible for providing high-quality physical therapy services to our residents, including:

• Evaluating and treating residents with a wide range of physical disabilities, injuries, and illnesses

• Developing and implementing individualized treatment plans to achieve optimal functional outcomes

• Collaborating with interdisciplinary teams to ensure coordinated care and effective communication

• Maintaining accurate and detailed records of treatment plans, progress, and outcomes

Responsibilities:

• Provide Physical Therapy services to residents, including evaluations, assessments, and treatments

• Develop and implement individualized treatment plans to achieve optimal functional outcomes

• Collaborate with other healthcare professionals to ensure comprehensive care

• Maintain accurate and detailed records of treatment plans, progress, and outcomes
