PRN Physical Therapist (PT)

Shippenville Nursing and Rehab – Shippenville, PA

Shippenville Nursing and Rehab is seeking a compassionate and motivated PRN Physical Therapist (PT) to join our in-house rehabilitation team .

This is a great opportunity for a Physical Therapist looking for flexible PRN hours while working in a supportive Skilled Nursing and Long-Term Care (SNF/LTC) environment. Help our residents improve mobility, regain independence, and achieve their highest level of function.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans

• Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence

• Perform evaluations, progress assessments, and discharge planning

• Monitor resident progress and modify treatment plans as appropriate

• Complete accurate and timely clinical documentation using Net Health and PointClickCare (PCC)

• Collaborate with nursing, therapy, physicians, and other members of the interdisciplinary care team

• Educate residents, families, and caregivers regarding treatment plans, safety, and mobility strategies

• Promote resident independence, safety, and overall quality of life

Qualifications

• Graduate of an accredited Physical Therapy program

• Current or eligible Pennsylvania Physical Therapist license

• Strong clinical, communication, and interpersonal skills

• Ability to work independently while collaborating with an interdisciplinary team

• SNF/LTC experience is preferred, but not required
