The Home Care RN Case Manager conducts nursing visits to include admissions, revisits, and discharges while providing assessments and education to patients. The Home Care RN Case Manager for Saint Elizabeth Home Care is a part of a collaborative interdisciplinary team with ample administrative support. Our Home Care RN Case Manager is equipped with point of care devices (iPad), as well as all necessary and requested medical equipment and supplies.Qualifications for a Home Care RN Case Manager:Current unencumbered RN license in Rhode Island.Minimum of one year nursing experience, home care experience preferred.
