Work Environment

This position is based in a home care setting, where you will conduct therapy sessions at patients' residences. Key Responsibilities

• Assess patients' physical conditions through clinical evaluation, history-taking, and examination to determine their specific therapy needs.
• Create individualized treatment plans that incorporate evidence-based techniques, therapeutic exercises, and home programs tailored to each patient's limitations and goals.
• Implement hands-on therapies, including manual therapy techniques, modalities, and functional exercises to promote recovery and mobility.
• Employ independent judgment in modifying treatment strategies based on patient responses and progress.
• Maintain comprehensive documentation of patient evaluations, care plans, and progress notes in accordance with regulatory standards and best practices.
• Engage in ongoing communication with patients and their families, addressing questions and concerns regarding their treatment modalities and expected outcomes.
• Educate patients about injury prevention and recommend lifestyle modifications to assist in their recovery.
• Collaborate with other healthcare providers as necessary to enhance patient treatment efficacy.

Patient Population / Client Focus

Your role as a Physical Therapist will focus on diverse patient populations ranging from those recovering from orthopedic injuries to individuals coping with post-surgical complications. You will primarily work with adults and the elderly, including individuals with chronic conditions, neurological disorders, and mobility impairments. Building rapport with patients through empathy and active listening is crucial in promoting adherence to treatment plans.

Qualifications

• A Bachelor's degree in Physical Therapy (or equivalent).
• A valid and current Physical Therapist license in the state of Florida.
• A minimum of three years of experience in physical therapy or related fields, with a preference for home care experience.
• Strong clinical evaluation and problem-solving skills.
• Exceptional interpersonal communication abilities and a dedication to patient-centered care.

Compensation and Benefits
