Responsibilities
- Perform initial physical therapy evaluation and establish a plan of care for residents in the LTC setting.
- Deliver evidence-based therapies to improve mobility, strength, balance, gait, and functional independence.
- Reassess residents regularly and modify treatment plans based on progress and goals.
- Document assessments, treatment interventions, and outcomes in patient records in a timely, compliant manner.
- Collaborate with nursing staff, therapy colleagues, and care teams to coordinate care and support discharge planning as appropriate.
- Educate residents and families on exercise programs, safety strategies, and Activity of Daily Living (ADL) optimization.

Requirements
- Active Physical Therapist license in Texas (or eligible for licensure)
- Degree in Physical Therapy (Doctor of Physical Therapy preferred)
- Ability to work in a long-term care setting
- Prior clinical experience in physical therapy; emphasis on mobility, functional training, and fall prevention
- CPR certification or ability to obtain prior to start
- Must be legally eligible to work in the United States
