Avenue360 Health and Wellness is seeking a Housing Case Manager in St. Louis, MO to provide case management and supportive services for participants in permanent supportive housing programs. This role aims to ensure housing stability and support clients in navigating housing challenges. The ideal candidate should possess a Bachelor’s degree in social work or a related field and have at least 2 years of case management experience.
