What does a Transitions at Home RN Case Manager do? Transitions At Home Registered Nurse Case Managersprovide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
